What is Genetic Testing

Genetic testing, also known as Preimplantation Genetic Testing (PGT). Genetic Testing (PGT) is a technique used during IVF to examine an embryo’s chromosomes and detect specific genetic abnormalities. Using advanced technology like next-generation sequencing, PGT helps identify healthy embryos before transfer, increasing the chances of a successful pregnancy and reducing the risk of genetic disorders. Genetic testing are three main types:

PGT‑A for aneuploidy (incorrect chromosome number).
PGT‑M for monogenic (single gene) disorders.
PGT‑SR for structural chromosome rearrangements.

This testing helps ensure that only healthy embryos are selected, reducing the risk of genetic diseases and increasing the likelihood of a successful pregnancy.

How Genetic Testing Works in the IVF Process?

Genetic testing is an advanced step integrated into the IVF process to analyse embryos before they are implanted. Here's how it typically works:

1. Ovarian Stimulation & Egg Retrieval

The IVF process begins with hormone medications to stimulate the ovaries to produce multiple eggs. These eggs are then retrieved in a short, outpatient procedure.

2. Fertilization in the Lab

The retrieved eggs are fertilized with sperm in a lab to create embryos. These embryos are monitored closely as they develop.

3. Embryo Development to Blastocyst Stage

Over the next 5–6 days, embryos grow to the blastocyst stage, which is when they have enough cells for safe testing without harming development.

4. Embryo Biopsy

At the blastocyst stage, a few cells are carefully removed from the embryo’s outer layer (which becomes the placenta). This procedure does not affect the part that will develop into the baby.

5. Genetic Analysis

The biopsied cells are sent to a specialized genetics lab. Using advanced techniques like Next-Generation Sequencing (NGS), the lab screens for chromosomal abnormalities (PGT-A), specific gene mutations (PGT-M), or structural rearrangements (PGT-SR).

6. Embryo Freezing

While testing is being completed (which usually takes about a week), embryos are cryopreserved (frozen) to preserve their quality.

7. Embryo Selection & Transfer

Once results are available, the fertility specialist selects the healthiest embryo(s) based on genetic results and prepares the patient for frozen embryo transfer (FET).

Limitations and Ethical Considerations

While genetic testing in IVF offers significant benefits, it also has limitations. PGT cannot detect every genetic or chromosomal issue, and while the risk of misdiagnosis is very low, less than 2% but it is not zero. The test does not guarantee the birth of a healthy baby. Because PGT doesn't assess every chromosome in detail, prenatal testing during pregnancy (such as amniocentesis or chorionic villus sampling) is still strongly recommended to confirm chromosomal structure and number.

In some cases, PGT may not yield results for certain embryos, or no chromosomally normal embryos may be available for transfer. This is more common in older patients, as the likelihood of chromosomal abnormalities increases with age.

Beyond clinical limitations, genetic testing also raises ethical considerations. Fact sheet to understand what is involved in the frozen embryo transfer process

What is a frozen embryo transfer cycle?

A frozen embryo transfer (FET) cycle means thawing one or more embryos (frozen during a previous IVF treatment cycle) and transferring that embryo (or embryos) to the uterus in order to try to establish a pregnancy.

When is a frozen embryo cycle recommended?

When ovaries are stimulated in a typical IVF or ICSI cycle, they produce multiple eggs for insemination and this often leads to a number of healthy embryos being created. Fresh embryo transfer may not be appropriate, or if the fresh embryo is transferred to the uterus on the 6th day of egg retrieval, the rest can be frozen to use if the first transfer does not result in a pregnancy, or at a later time to create a sibling.

It’s not uncommon for practice to freeze all embryos after an IVF cycle and not to transfer any immediately if the lining of the uterus or hormone levels are not suitable or if a patient is at risk of developing ovarian hyper-stimulation syndrome (OHSS).

It is also increasingly common for the fertility specialist to recommend that we freeze all of a patient’s embryos because the more natural environment in the uterus during a later cycle may have a better chance of establishing a pregnancy.

Freezing embryos is also a potential avenue for a woman who wants to postpone pregnancy until later times like in her late 30s or early 40s but who understands she may not have suitable eggs of her own by then.

What are the success rates of frozen embryo transfer?

The pregnancy or live birth success rate depends on a number of factors, including the woman’s age and quality of the embryo.

Frozen embryo survival rates are more than 90% if embryos are frozen by ‘vitrification’ at the blastocyst stage (5-6 days after fertilization).

If they are frozen by ‘slow freezing’, approximately 80% survive the freezing and thawing process (old method).

If an embryo vitrified at the blastocyst stage is thawed and transferred, it has approximately the same success rate as a fresh embryo. There is no evidence that frozen and thawed embryos result in a greater number of miscarriages or abnormalities.

What does a frozen embryo transfer cycle involve?

When you are ready to begin a FET cycle, contact your clinic/IVF coordinator. There are a few necessary steps before FET treatment can be started.

Before a frozen embryo can be transferred into your uterus, the lining of the uterus (the endometrium) must be prepared to the right stage in either a ‘natural cycle’ or a ‘programmed aka hormone replacement therapy cycle.’

Natural cycle

In a natural FET cycle, we allow the endometrium to thicken naturally as part of your normal cycle. We monitor the cycle by checking the developing egg follicle on the ovary and the thickness of the endometrium by ultrasound, and once they have reached an appropriate size/thickness, we trigger ovulation, usually with a Pregnyl injection.

The embryo transfer then takes place approximately 6-7 days after this. The exact timing will depend on your individual treatment plan, your hormone levels on the day of ovulation trigger, and at what stage of development your embryos were frozen. Progesterone medication, either as an injection or pessaries, is often given during the second half of the cycle to help support the endometrium.

Hormone replacement therapy (HRT) cycle or Programmed cycle
An HRT cycle is often used to control the cycle, particularly in women who don’t have a regular cycle. We use additional estrogen and progesterone medications to develop the endometrium to the right stage.

You will take estrogen medications throughout the cycle and progesterone medications during the second part of the cycle. We monitor the development of the endometrium by ultrasound and estradiol hormone level to check when to introduce the progesterone. Your embryos are normally implanted on the 6th or 7th day after starting progesterone, but this can vary depending on individual circumstances and the stage at which your embryos were frozen.

When will my embryos be thawed?

The laboratory will thaw your embryos so that the stage of embryo development corresponds to the right stage of your menstrual cycle. Timing also depends on what stage the embryos were frozen. At Precision IVF, we usually freeze embryos at the blastocyst stage and then thaw them on the day of your embryo transfer.

How are embryos transferred?

We use a fine soft tube (a catheter) to pass the embryo through the cervix and into the uterus under ultrasound guidance. A small amount of the culture medium is also placed in the uterus with the embryo. This technique usually doesn’t need sedation, and most women only report mild discomfort.

What happens after the embryo transfer?

Before you go home after the transfer, your doctor and IVF coordinator will go through the instructions, including your medications and any lifestyle considerations.

We order a pregnancy blood test for 10 days after your embryo transfer. The blood test measures the hormone human Chorionic Gonadotropin (hCG), which is produced by the pregnancy. Please wait until the advised date to have your test as hormones used during the cycle can lead to false readings if you test too soon.

Whichever type of FET cycle you are having, it is very important to continue the prescribed medications until you have received the results of your pregnancy test.

Some women experience bleeding or spotting before their pregnancy test. This is not necessarily a period, and it is very important that you continue to take any prescribed medications until you have spoken to your IVF Coordinator or fertility specialist.

A Step-by-Step Guide to Intrauterine Insemination (IUI)

What is IUI?

An IUI is also known as artificial insemination in which sperm is placed directly into your uterine cavity near the time of ovulation. This procedure can be performed either with your partner’s sperm or with donor sperm.

How does the IUI procedure work?

IUI is considered the first-line treatment for unexplained infertility, mild endometriosis, or mild male factor infertility .

In general, IUI does two things differently than natural attempt to conceive: ovary stimulating medication (taken before the “HCG trigger injection”) help stimulate the growth of follicles (each follicle has an egg in it), making a woman more fertile than she would be in a natural cycle (without the medication). 2nd, the injection of sperm that is placed inside the uterine cavity, avoiding having to swim through the cervical opening and into the uterus on its own, allowing it to meet the egg more easily and at higher concentration. This is especially useful when the sperm count is low or the sperm do not move well. IUI bypasses the vagina and cervix, hence overcoming incompatibility between sperm and cervical mucus if there is any.

During the procedure, the male partner will be asked to provide a semen sample about two hours before being scheduled for insemination. This is done by masturbating into a sterile container at the doctor’s office or at home. The semen is “washed” to separate the sperm from the seminal fluid. This must be done before the sperm can be injected into the uterus because the seminal fluid contains substances that can irritate your uterus. Sperm “washing” may also improve the ability of the sperm to fertilize the egg.

The insemination takes only a few minutes. Your fertility specialist will place a speculum inside the vagina, insert a small soft catheter through the cervix into the uterine cavity, and inject the sperm into the uterus. IUI is not a painful procedure, you may experience slight discomfort from speculum placement.

Who are the ideal candidates of IUI?

Women with unexplained infertility
Couples where the male partners have minor semen abnormalities or ejaculatory dysfunction
Couples where male partners froze sperm prior to surgery, travel, or treatment for diseases that may affect his sperm production.
Women who may not ovulate, such as those with PCOS , or other reasons of anovulation like hypothalamic amenorrhea.
Anyone who needs donor sperm, such as LGBTQA+ couples, heterosexual couples with severe male factor who are not able to do an IVF cycle, or single women by choice

How much does IUI cost?
IUI is much less expensive than in vitro fertilization (IVF) and is many times covered by commercial insurances, making it a much more accessible and affordable option. IUI is also much less involved and invasive compared to IVF. The average cost of an IUI cycle is around $800.

What preparation do you need to do for an IUI?

Before the IUI procedure, you’ll need to do some testing to ensure that IUI is the right treatment for you. It involves the following fertility test, all these can be done in 1 visit prior to IUI:

Ovarian reserve test- by antral follicle count (AFC) done using ultrasound and/or a blood test called anti-Mullerian hormone (AMH) level.
Your doctor may suggest you to do genetic blood work to find out whether you’re a carrier for any particular genetic mutation.
You’ll have a hysterosalpingogram (HSG) to ensure your fallopian tubes are patent.
Semen analysis

Once your basic fertility work up is complete and you are ready to start the IUI treatment cycle, you may receive one of the following medications to grow follicles:

Clomiphene citrate (Clomid) and letrozole (Femara) are oral medications commonly used for IUI cycles. Occasionally your doctor may advise you to take FSH and/or menopur, which are injectable medications for ovarian stimulation. Clomid or Femara are taken orally for 5-7 days typically starting on day 3 of menstrual cycle. In order to appropriately time for this treatment, your ovulation is carefully monitored by midcycle ultrasound and bloodwork. With ultrasound, the uterine lining thickness and follicles are measured, and with blood work, we test estradiol and progesterone. Based on these results, you will be advised to get an HCG injection (“trigger shot”) to help in ovulation and the timing of the IUI procedure. IUI is typically scheduled 1-2 days after ovulation begins. For IUI to be effective, it must be performed before or within 24 hours of ovulation, or the egg is no longer fertilizable. The life of an egg is much shorter than the life of the sperm, so there is only a short window of time in which an egg and a sperm can interact and create an embryo. Hence, it is better for the sperm to be in the uterus early rather than late.

Male partner should not ejaculate for 1-2 days before the procedure to maximize sperm count available for IUI. Check with your doctor to see if there are any other recommendations specific to your care.

What are the chances of twins or triplets with IUI?
Because these ovary stimulating medications cause many follicles to mature and ovulate, leading to a higher risk of twins, there are about 10-12 percent chances of twins. Triplet pregnancies are rare with oral medications. Midcycle ultrasound monitoring helps in reducing the risk of multiple pregnancies by knowing how many mature follicles you have. Your doctor may advise you to avoid IUI if you have more than 3 mature follicles on monitoring ultrasound in a particular cycle. The dose of medication can be adjusted to optimize the response and reduce the risk of twin pregnancy.

Will I need multiple IUIs to become pregnant?

The success rate of IUI is typically lower than the IVF cycle. It depends on many factors like ovarian reserve, women age, and sperm concentration. If you don’t become pregnant after an IUI cycle, you may repeat the procedure. Further evaluation or other treatment options such as IVF, may be recommended if a patient does not get pregnant after several cycles.

When you will I know if IUI treatment is successful?

You will know in 2 weeks! if you become pregnant, you will most miss your next period. A blood test will confirm whether or not you are pregnant.

What Is Age-Related Decline in Fertility?

It is well established that your chances of getting pregnant are highest when you are young, in your 20s and early 30s. Your egg reserve is abundant, and egg quality is healthier, increasing your chances of having a healthy baby. Fertility declines gradually throughout your adult life, even in your 20s. By age 30, the chance of natural pregnancy is about 20% per month.

Fertility declines with age due to a decrease in egg count, as we women are born with a fixed number of eggs, and we have been losing them, which started even before we are born. With age, there is a decrease in egg quality and higher chances of chromosomal abnormalities. This decline in egg count and quality is much steeper in a woman's late 30s. That's where you might have heard the steepest age-related fertility decline after age 35.

What Is Egg Freezing?

With egg freezing, the goal is to maximize your fertility before it begins to decline. Egg freezing or oocyte cryopreservation is a process in which a woman's eggs (oocytes) are extracted, frozen, and stored as a means of possible conception in the future. The first human birth from a frozen oocyte was reported in 1986. Oocyte cryopreservation has significantly advanced over the past few years, with the improved overall success of eggs surviving the freezing and thawing process. It is no longer considered an experimental procedure by the American Society for Reproductive Medicine. The techniques leading to enhanced gamete survival, potential fertilization, and live birth rates allow women a much greater degree of autonomy than was possible several years ago.

Unlike fertilized egg freezing (embryo cryopreservation), egg freezing does not require sperm since the eggs are not fertilized beforehand. With egg freezing, multiple follicle growth is stimulated with injectable gonadotropin medicine; each follicle has an egg in it. Unfertilized eggs are then collected from your ovaries by an outpatient minor surgery, and oocytes are frozen for later use. When you are ready to begin the process of conceiving, the process of fertilization of the egg and transfer of the embryo is done. Keep reading for a more detailed discussion about the process.

The Process of Freezing Eggs

A woman who opts to freeze her eggs can expect the process to take 2-3 weeks. Here's an overview of the process.

Consultation with a Fertility Specialist

Our fertility specialist, Dr. Sushila Arya, will start by reviewing your medical history in detail with a focus on fertility. As part of the assessment, she will examine your menstrual cycle for regularity and assess your ovarian reserve to determine your potential oocyte yield and the correct medication dosage.

Hormonal Injections and Monitoring

Gonadotropin hormone injections [usually FHS (Follistim or GonalF) and LH (Menopur) injections daily for about 10-12 days] will be given to the patient to stimulate the growth of follicles in ovaries that leads to maturation of the eggs. Each follicle has an egg in it.

Monitoring of the cycle is done by ultrasound and blood hormone tests. On average, it involves a total of 5-6 clinic visits. This monitoring allows dose adjustment of gonadotropin to maximize response and egg yield. It takes about 10-12 days to grow follicles, and egg retrieval takes place at the end of this stimulation phase.

Once follicles are mature (15-20 mm), you receive a final hgc or Lupron injection ("trigger shot") to help egg maturation. Dr. Arya will perform an egg retrieval procedure. Egg retrieval takes less than 20 minutes under mild anesthesia or sedation. Through ultrasound guidance, the oocytes and the surrounding fluid in the ovarian follicles are aspirated vaginally. The maturity of the eggs is assessed under the microscope, and those that are mature are cryopreserved.

Freezing Eggs

Following egg retrieval, the embryologist isolates and cryopreserves the egg using a freezing solution (cryoprotectant) in the embryology lab (connected with the procedure room). Currently, vitrification is the method of choice for cryopreserving oocytes, and this is achieved by ultra-rapid cooling into liquid nitrogen where they can be stored. This method has allowed better post-thaw survival of eggs and improved success in achieving live birth using frozen eggs.

In Vitro Fertilization (IVF)

When the woman is ready to use the frozen eggs to achieve pregnancy, these cryopreserved eggs are placed in a warming/thawing solution and reassessed. Those eggs that survived the freezing process are fertilized with intracytoplasmic sperm injection (ICSI), where a single sperm is injected directly into the egg. The fertilized eggs will grow in culture until the embryo(s) are ready to be transferred into the uterus to achieve pregnancy, typically 3 or 5 days after fertilization. Eggs can be cryopreserved for a long time.

Who are the Candidates for Egg Freezing?

Cryopreservation of the oocytes can be considered for a variety of reasons:

Women with cancer who require chemotherapy and/or pelvic radiation therapy that may affect fertility.
Surgery that may cause damage to the ovaries
Risk of premature ovarian failure because of chromosomal abnormalities (e.g., Turner syndrome, fragile X syndrome) or family history of early menopause.
Ovarian disease with risk of damage to the ovaries such as endometriosis
With a progressive disease like sickle cell disease or renal disease or those suffering from an autoimmune disease such as lupus should also consider egg freezing.
Genetic mutations that require removing the ovaries (e.g., BRCA mutation)
Fertility preservation for social or personal reasons to delay childbearing
Furthermore, some women who wish to have children alone or with a partner of the same sex may also elect to freeze their eggs and use a sperm donor. Alternatively, transfer of an embryo into another woman's uterus ( gestational carrier ) to carry the pregnancy for you if that is medically necessary.
Females transitioning to male want to freeze eggs before getting reconstructive surgery or hormone therapy. Most of these treatments may result in a partial or permanent loss of fertility.

Risks and Side Effects Associated with Egg Freezing

Risks that come with egg freezing are rare. Risks are similar to those associated with ovarian stimulation for IVF, including small risks of ovarian hyperstimulation syndrome (enlargement of the ovaries and fluid accumulation in the pelvis and abdomen), infection, and bleeding related to the egg retrieval procedure. Additionally, it is important to remember that egg freezing may improve the chances of a future pregnancy. Still, it does not guarantee 100% success, so there is a risk of emotional distress.

DTaP vaccine (Tetanus, Diphtheria, Pertussis) and flu vaccines are routinely recommended for pregnant women. It can protect you from infection and make your newborn immune to infection before they get immunized.

This is crucial because flu and whooping cough are especially dangerous to young children. Don't get the influenza vaccine through the nasal spray since it contains live viruses. At your initial appointment, I will discuss your (and your partner if applicable) thorough medical history, surgical history, family health and genetic history, and any previous testing or treatments if you had. Pieces of the puzzle that need to be right to achieve pregnancy are:

Ovulation: Are you ovulating regularly. Ovulation requires the growth of a follicle and communication between your brain, pituitary gland, and ovaries. Once the follicle is mature in size, your pituitary gland (a gland in your brain) talks to your ovaries and releases the LH hormone. The spike of LH hormone triggers series of events in your ovary/follicle and helps in the final maturation and release of the oocyte (egg) from the follicle. Off note, only a mature egg will be fertilized by sperm. Ovulation occurs somewhere between 28 to 36 hours after the LH peak. How do we test ovulation?
Blood progesterone level at the appropriate time in your cycle. If your menstrual cycles are regular, you most likely have regular ovulation? When necessary, we can confirm your ovulation by testing your progesterone about a week before your period.
Ovulation predictor kits (OPKs) are another way to test ovulation. It works by detecting the LH (luteinizing hormone) levels in your urine. LH ovulation kits may not be accurate all the time and for everyone. We will discuss the appropriate use of OPKs if needed.
If you have been charting your ovulation using OPKs or other menstrual tracking apps, please bring those with you. It may help plan or defer testing for ovulation in your case.
Anatomy of uterus and ovaries: We prefer transvaginal ultrasound whenever feasible to assess your uterus and ovaries. I will examine the lining of your uterus, any evidence of fibroids or adenomyosis. In addition, I will assess your ovarian reserve with ultrasound and check for possible ovarian cysts or other concerning findings.
We will assess if you will need a hysterosalpingogram (HSG). HSG is a specialized X-ray test that I use to evaluate the patency of fallopian tubes.
Semen analysis (SA) is done to assess for male infertility. SA is done by taking an ejaculated semen sample and evaluating it in the laboratory. In SA, we assess the following parameters: semen volume, sperm motility, concentration, and morphology.
I acknowledge you or your partner may possibly feel awkward and anxious with SA. I want to assure you that overall, this is a straightforward process, and it is a vital part of infertility assessment. I recommend SA must be done as a part of the initial evaluation.
After applicable infertility evaluation, I will educate and guide you on the testing results ideal fertility treatment plan for you, tailed to your condition. Overview of Infertility

Infertility is defined as not being able to conceive despite having frequent, unprotected sex for at least a year. There may be issues with you or your partner, or a combination of factors may contribute to it. In general:

One-third of the cases involve infertility issues for the woman
In the remaining cases, both the woman and the man are infertile, or no cause can be determined

First, a woman's body must release an egg from one of her ovaries (ovulation). Second, the egg must travel through a fallopian tube on its way to the uterus (womb). The next step is for the sperm to join the egg (fertilize it). Lastly, the fertilized egg must implant into the uterus (implantation). A problem with any of these steps can lead to infertility. All steps during ovulation and fertilization must take place correctly to achieve pregnancy.

Many factors can affect fertility, both for men and women. Here are the common risk factors for both genders.

Fertility declines as we age (over age 35 for women or over 40 for men)
Abuse of substances.

Exposure to environmental toxins such as lead

What Can Cause Infertility in Women

Apart from the common risk factors among men and women, here are the causes of infertility in women:

Endometriosis or growth of endometrial tissue outside your uterus prevents the egg from reaching the fallopian tube
Ovulation disorders and hormonal imbalances interfere with the production and release of eggs. These include hormonal conditions such as polycystic ovary syndrome (PCOS)

High prolactin / Hyperprolactinemia -Overproduction of prolactin by the pituitary gland can reduce estrogen production and lead to infertility.
Amenorrhea or the absence of periods caused by extreme physical exertion or deficient body mass index
Scarring binding organs that can develop from past appendicitis, pelvic infection, endometriosis, and abdominal or pelvic surgery
Pelvic inflammatory disease (PID)
Chronic diseases, such as Lupus, arthritis, asthma & hypertension

Diagnosis of Infertility in Women

A fertility physician will begin by taking an in-depth medical and fertility history. Afterward, they will perform a complete physical examination and an ovulation test. A number of these tests following the first two steps can help diagnose or rule out a problem with female fertility:

Blood testing to check thyroid and hormone levels
Pelvic exam, including a Pap smear
Transvaginal ultrasound – For assessing the structure of the uterus, tubes, and ovaries and useful to detect fibroids, polyps in the uterus, fallopian tube obstruction, and ovarian cysts.
Laparoscopy helps determine if there are problems such as endometriosis, fibroids, or scarring.
Hysteroscopy: A specialist inserts a tube with a light through the vagina to examine the uterus.
Hysterosalpingogram (HSG): The test looks for blockages. X-rays are taken during the injection of a dye through the fallopian tubes.
The saline sonohysterogram (SIS) involves filling the uterus with sterilized salt water and examining it with a transvaginal ultrasound. The uterus is easier to view this way.

Infertility in Men

In cases where male infertility is suspected, the focus is generally on analyzing the semen to determine whether a sperm has successfully met an egg. Part of the male infertility testing includes the number of sperm, their mobility (sperm motility), and their shape.

The presence of slightly abnormal semen does not necessarily indicate infertility. By analyzing semen, an infertility specialist can determine which factors may cause infertility.

What Can Cause Infertility in Men

Low testosterone (hypogonadism) or low sperm count
An injury to the testes can lead to a decrease in sperm production.
History of testicular or prostate disorders
Varicocele is an enlargement of the veins around a man's testicles, which causes them to overheat and affects the shape and amount of sperm.
Hormonal imbalance, resulting in insufficient testosterone production
Ejaculation problems particularly delayed ejaculation or retrograde ejaculation (semen goes back into the bladder)
Undescended testicle
Medical conditions such as diabetes, Cystic Fibrosis , and other autoimmune disorders
A genetic problem with the absence of the main sperm pipeline known as the vas deferens

Diagnosis of Infertility in Men

The first step is for your doctor to take a complete medical history. As part of this process, the doctor will ask about the overall condition of your health, sexual history, and anything else that might affect your fertility. You will also undergo a physical examination where they will examine your genitals for lumps and anomalies. An analysis of the semen will follow. If your doctor feels you require additional testing, they may order the following:

Semen Analysis to check for sperm problems, including low counts and poor mobility.
Blood Testing to determine testosterone, hormone, and thyroid levels.
Ultrasound of the genital area: A scrotal ultrasound detects varicocele and other testicular disorders.
Genetic testing to check for chromosomal abnormalities.
